IN-SIGHT is an anonymous harassment, discrimination, or bias/hate incident submission form which allows members of the Queen's community to share incidents of harassment, discrimination, hate, and violence targeting personal characteristics protected under the Ontario Human Rights Code (e.g., race, religion, sexual orientation, disability) which have been experienced or witnessed.
The purpose of IN-SIGHT is to gather anonymized aggregate statistical information about incidents occurring on/off campus. The platform offers a way for Queen’s community members (students, staff, faculty etc) to anonymously share information about harassment, discrimination, or bias/hate concerns so that the information can be used to identify systemic trends.
Please note that IN-SIGHT is NOT a formal reporting and complaint mechanism.
If you wish to make a formal report and/or complaint under the Harassment and Discrimination Prevention and Response Policy (2021), DO NOT use the IN-SIGHT tool. Please use the Harassment & Discrimination Prevention & Response Policy Submission Form.
You are not required to provide your name or contact information to share information using IN-SIGHT. However, if you would like a Human Rights Advisory Services advisor to follow-up with you about your experiences and advise you of your options, IN-SIGHT directs you to the Human Rights Client Intake Form.
IN-SIGHT anonymous submission form and Human Rights Client Intake Form are submitted to the Human Rights and Equity Office: Human Rights Advisory Services, and received by staff Monday to Friday, 8:30am to 4:30pm during our regular business hours.
Please note that while this tool does NOT ask for/require your name or contact information, if you choose to enter that information into any of the textboxes in this tool, that information is included in your form submission. Nevertheless, even if identifying information is accidentally given, we will still do our best to maintain your confidentiality and privacy, by anonymizing the information provided as necessary.
Information from anonymous submissions collected through IN-SIGHT is used to help the university generate a more accurate picture regarding how harassment, discrimination, or bias/hate related incidents affect Queen's community members, and also helps identify trends (new and recurring) that are used to inform policy, programming, services and future anti-racism and other discrimination/harassment prevention and response initiatives at Queen's University.
Your participation in IN-SIGHT allows the collection of data about hostilities directed toward marginalized groups at Queen's, including disaggregated data on issues affecting particular communities (e.g., anti-Black/Indigenous/Asian/racism, Islamophobia, Antisemitism, homo/transphobia, ableism, microaggressions, etc.) Aggregated anonymized statistics from IN-SIGHT are published by the university each year. Anonymized narrative information, for example about the impact of incidents on community members, may also be shared as part of this reporting. This yearly report is generated by the Human Rights & Equity Office and is accessible to all members of the Queen's community.
Your confidentiality and trust are extremely important to us and we will make every effort to respect and preserve your privacy. Please be aware however, that in certain circumstances we may not be able to guarantee complete anonymity.
Where necessary, information submitted anonymously through IN-SIGHT may be shared with university or other officials who, due to legal and policy obligations, may be responsible for campus safety as well as overseeing investigation and/or resolution of harassment, discrimination, or bias/hate incidents. (Note: The University is limited in how it addresses specific submissions that are anonymous.)
Legal and policy obligations relating to campus safety generally include;
- Where there are reasonable grounds to believe that an individual is at risk of self-harm;
- Where there are reasonable grounds to believe that an individual may pose a threat or risk to members of the University Community.
Nonetheless, regardless of the situation, matters are handled discretely and in conformity with University privacy policies.
Anonymous submissions via IN-SIGHT are NOT the same as reporting an incident or making a complaint. Personal identifiers (e.g., name, address, telephone number) are not required. In addition to, or instead of, completing the anonymous submission form you may contact the Human Rights and Equity Office by submitting a Human Rights Client Intake Form.

Submitting a Client Intake Form, you understand:
- That you will confidentially share your contact information with the Human Rights Advisory Services (HRAS) to arrange an appointment with an advisor to discuss your situation;
- That consultations with the HRAS are confidential, except in circumstances where information must be shared to address a risk to the health and safety of members of the University Community (e.g., where there are reasonable grounds to believe that an individual is at risk of self harm; there are reasonable grounds to believe that an individual may pose a threat or risk to members of the University Community.)
- That client intake form submissions are received by Human Rights Advisory Services Monday to Friday, 8:30am – 4:30pm and that you will be contacted to arrange an appointment during these regular office hours.
