This form is to be used for any of the following:

  • To delegate or revoke Shopper, Requester, or Reviewer access to acQuire for NON-EMPLOYEES
  • To change or revoke Shopper, Requester, or Reviewer access to acQuire for EMPLOYEES
  • To report issues with acQuire to the system administrator
