Email & Calendar

 

1. Request a Calendar Resource (Add/Change/Delete)

  • These requests will add, change or delete a resource (such as a room) in the calendar  system. It can also be used to transfer ownership of a resource to another person.

2.  Request a Distribution Group 

  • Distribution groups are lists that can be used for scheduling departmental or group meetings. Use this form to request that your distribution group be added to the Global Address List (GAL) that can be found within Microsoft Outlook and the Outlook Web App.

3. Request a Shared Mailbox (i.e. mydepartment@queensu.ca)

  • These requests will add, change or delete a shared mailbox (such as mydepartment@queensu.ca) that can be accessed by multiple people in a department.

4. Request a NetID or Email Account

  • These accounts are for individual use by Queen's staff or faculty. This form is used for a new staff or faculty member and new email accounts will be set up on the Exchange server.

5. Request a NetID password change

  •   Use this form to request a password change/reset for a faculty of staff member.

6. Request a Contingent Worker Account  

  • Please  do not  use this form for new staff or faculty.  This is for a guest/consultant/visitor who absolutely will NOT be issued any type of appointment and is not paid by the university

    7. Request a Generic/Departmental Account

    • Please do not use this form for new staff or faculty.  This is for a project/lab/position type of generic naming that is not attached to a particular individual.

    8. Disable NetID Request form

    • In the event a NetID must be disabled immediately please fill out this form and verify the instructions by calling 78888 (the request must be through the form to have a written record of the request)

    Other

    1. Request a Departmental Contact Information Change

    • Please let us know if the business phone, location or departmental information is incorrect for your department.

    2. TSM Accounts or Password Reset Requests

    • Please use this form to request forgotten node ids or users TSM password resets.

     

     

    Moodle

     

    1. Request a Moodle Guest Account or Test Account

    • Please use this form to request a GUEST account for a Moodle course.
    • Please use this form to request a TEST account for a Queen's employee to use with Moodle.

    Streaming

     

    1. Request a Departmental Streaming Account  

    • Use this form to request a streaming account for your department. Do not use this form to request a personal account. A personal account is created the first time an individual logs into stream.queensu.ca.

    Telephone Addition

    • There is a one-time installation charge.
    • Fax machines and modems require an analog line.
    • Most telephones are installed on digital lines.
    • This request will be completed within 5 business days.
    • $110.00 installation charge

    Monthly charges:

    • line charge - $21.09/month
    • equipment - $4.55/month (standard telephone set)
    • DID (direct-inward dial) number - $8.21/month. There is no charge for internal numbers (i.e. extensions that begin with 7).

    Ethernet (Network Jack) Addition

    Telephone or Ethernet Change

     

    Request a Change to an Existing Telephone or Ethernet Service

    • There is no charge for feature changes unless a phone upgrade is required. You will be advised of this at the time of the request.

     


    Telephone or Ethernet Move

     

    Request a Service be Moved from One Location to Another

     

    When rearranging space within your department, we recommend that individuals who are doing the same job (but changing their location) keep their extension. If the individual is moving to another job or going on maternity leave, we recommend leaving the phone number for their replacement.

    • A technician is required to move the service; you cannot move it yourself.
    • If you require assistance with a larger telecom related project please use these forms to provide contact information and give a brief description of your requirements in the description field.  Telecom staff will contact you for further discussion if required.
    • This request will be completed within 5 business days.
    • There is no charge unless a new jack installation is required.
