Claims must be received by Great-West Life within 180 days after the calendar year in which the charge was incurred. Late claims are not admissible.

How to submit a claim


GroupNet for plan members allows you to submit claims online. To learn how to register, visit the GroupNet for Plan Members section of the HR website. To submit a claim, log on to GroupNet for Plan Members using your username and password.

By mail

Once you have registered for GroupNet for Plan Members, log on using your username and password to access Medical, Dental or Medical Out-of-Country claim forms to print and submit to Great-West Life’s Claims Centre.

Completed claim forms should be sent to:

If you have not registered for GroupNet for Plan members and would like to submit a paper claim, general forms are accessible in the Forms section of the Great-West Life website.

Provider eClaims

Provider eClaims allows the provider to submit claims electronically at the point of service. It expands convenient dental-style electronic claim submission to other healthcare provider types, including chiropractors, physiotherapists and vision care providers.
